NVIDIA’s red team identifies three recurring LLM application security failures

Unsafe code execution, weak retrieval permissions and active output rendering connect model behavior to real application impact.

What happened

NVIDIA’s October 2025 guidance groups recurring findings from its AI red team into three practical areas: executing generated code, enforcing access to retrieval data and rendering model output. This is engineering guidance rather than a single new vulnerability.

A prompt becomes more consequential when an application can execute its output, retrieve information without checking the user’s permissions or load attacker-controlled resources while displaying a response. Each path needs controls outside the model.

Review execution isolation, enforce authorization at retrieval time and render responses safely. Input and output filters can support those controls, but should not be treated as a replacement for permission boundaries.

Next step

Review code execution, retrieval authorization and response rendering as separate boundaries.
